To our patients,
It is important in these times to be safe and follow the prescribed protocols as recommended by the CDC and stipulated by Governor Evers.
Prior to your visit and at the time of your arrival, we’ll be screening you for COVID-19 signs and symptoms, such as:
Chills, fever, cough, shortness of breath, headaches, sore throat, and loss of smell and taste.
If you demonstrate any of these or have been diagnosed with COVID-19 in the last two weeks, travelled by plane or train in the past two weeks, we will reschedule your appointment for a later date.
To maintain proper safety protocols, we require that patients and staff wear a mask and sanitize their hands upon arrival. Please come unaccompanied to your appointment unless it’s necessary to have ambulatory help or a guardian present.
It’s important for our office to maintain a sterile environment, this includes disinfecting commonly touched items such as surfaces, frames, displays, chairs and pens.
In order to reduce contact time with our patients and to assure the highest quality of care, an Optomap Wide-Field Retinal Photo will be done in place of dilation as part of your eye wellness exam. The fee for this benefit is $35.
All visits must be scheduled, this includes dispensing of glasses and contact lenses. We’ve spaced our appointments to minimize the number of people in the office at one time and to allow for the appropriate social distancing.
We want to continue to provide the best care while keeping everyone safe and look forward to welcoming back our patients, neighbors and friends. Thank you for your understanding, we appreciate your trust and loyalty.
